Simply fill out the form below with the name and email address of the person you referred (This information is required. Please reach out to your referral to obtain accurate information). Our system will check if they have an active account and update your Affiliate Dashboard accordingly.

PLEASE NOTE: This form is not for adding new referrals. This form is for adding a referral, who:

• May have signed up without using your affiliate link.

• Did not enter your Referral Information at Sign Up.

• You have referred in the past and would like to receive credit for under our new affiliate program commission structure.
